Backstop is currently seeking an experienced and committed assistant practitioner to work full time assessing the needs of elderly people and people with disability. This post is based in Southwark.
The ideal candidate will have an appreciation and understanding of current issues in social work practice. They will have experience of assessing and supporting elderly people and people with disability.
The pay rate for this locum practitioner job is up to £20 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Responsibilities of the reablement service assistant practitioner:
Following a contact, to establish the service user’s eligibility for assessment and determine the level of assessment required.
To work as part of the duty team and when appropriate offer advice, guidance and signposting.
To assist in undertaking assessments, working alongside other professionals and clients, maximizing their independence.
Identify safeguarding and quality of care issues. To ensure that clients’ welfare is protected and that the quality and standard of support provided is optimised.
Requirements for this post:
Ability to use IT systems to input and access data, use word processing systems and Email.
Good literacy and numeracy skills and ability to write factual reports to meet the needs of Agencies/ Departments, to maintain appropriate records including financial assessments and to assist people with their finances. To produce high quality assessments.
